HOW TO SPONSOR A FLAG:
To sponsor a flag, click the orange button below. it will take you to an online form. There is a lot of information you may submit about the Honoree that increases the Flag Tribute information, such as dates of military service, medals, and certificates. Take a look at the form, gather your info, then come back to submit. Alternately, you can download the PDF form and send in to our office once you complete it. Make sure you get as much information together as possible before you start completing the form as you will not be able to go back and add additional information once you submit the form. However, you can contact us directly and we can update the information for you on our end so that the Tribute Tag includes all the information you wish to have. You can also upload a photo--please limit to no more than two photos. Use the black button above to pay for your Flag Tribute. Once you submit your Flag Tribute you will be automatically directed to the payment page. If you are not, please return to this page and use the payment button above.
